How AI agents are streamlining pre-visit patient intake

A new patient checks in, but their chart holds only a name and phone number. Intake happens on a counter clipboard while phones ring and others wait. Reaching them beforehand was the plan, but daytime calls go unanswered while patients work, and by evening, the clinic is closed. So, charts remain empty until patients arrive.

Hello Patient changes that. Our new-patient intake page shows how agents books patients first—calling and texting at convenient times to collect essential details before the visit. Everything flows directly into your practice management system. While traditional workflows wait for patients to reach out, our solution handles proactive outreach, turning last-minute front-desk rushes into fully prepped, seamless check-ins.

You control the list and the window

Your team retains full control. Rather than automatically scanning your EHR, the AI agent works strictly from the list and timeline you provide. Outreach runs via phone and text—the channels best suited for reaching patients proactively—while web chat handles incoming website traffic.

Whether you target patients three days out or the afternoon before, you decide who gets contacted, when outreach begins, and how the schedule is managed. Nothing goes out without your explicit direction.

Information the agent collects

The agent collects required scheduling fields one at a time in a quick, natural conversation. It logs insurance details and confirms accepted plans, but leaves eligibility verification, benefit checks, and cost estimates to your staff.

Conversations remain strictly text and voice so no document uploads or card photos are required. If a patient asks something the agent can't answer, the full thread instantly routes to your team so the patient never has to repeat themselves.

Outreach finishes on the patient's time. A patient answering a text at 9 p.m. submits details that instantly log into your system, just like a midday entry. With no office hours and no callback queues, the agent works whenever your patients are ready.

What if a patient never responds?

Once the configured reminder cadence is complete, outreach stops automatically so patients aren’t spammed. If a patient replies “STOP,” they’re opted out immediately.

Patients who don’t respond can simply complete intake at the counter as usual—except now your front desk is handling one walk-in intake instead of six.

This is not a reminder system

Reminders confirm appointments, but AI agents complete records. Unlike one-way reminder texts that ignore patient questions, the agent answers inquiries, like confirming insurance, and keeps intake moving forward.

Proactive, two-way conversations deliver proven results. When Ortho1 Medical Group used an outbound agent for referral follow-up, they saw incredible results. The AI agent texted 718 referred patients, 426 replied, and by month two 57% of the patients who replied had booked.

The morning of the visit

When staff open the schedule in the morning, yesterday's six new patients are fully prepped—required fields are populated, insurance notes are logged, and zero re-keying is required. Everything integrates directly into your existing EHR, including ModMed, athenahealth, eClinicalWorks, and more.

What to ask a vendor about outbound intake

Does it run from a targeted list or read your EHR?

The vendor claiming its AI automatically finds gaps in your EHR is making a massive integration promise—ask to see it run live on your system before buying in. Ours is simple and list-driven: your team stays in full control of who gets contacted, every time.

What happens when a patient doesn't answer?

Ask vendors for their exact cadence, who controls it, and what stops it. Demand clear answers on opt-outs: the only right response is immediate, recorded, and permanent.

Where does collected data actually land?

Verify if information flows directly into your EHR or gets trapped in a third-party dashboard requiring manual re-entry. Always get compliance credentials in writing: HIPAA compliance, a signed Business Associate Agreement (BAA), and a SOC 2 Type 2 report.

Frequently asked questions

Do patients actually reply to a text about an upcoming visit?

Many do, because the text is about a visit they chose to book. The closest published numbers from Hello Patient come from referral outreach rather than intake: Ortho1 Medical Group, a multi-location orthopedic group, had an outbound agent text 718 referred patients, and 426 of them replied. By month two, 57% of the patients who replied had booked. A pre-visit text asks less of the patient than a referral text does, since the appointment exists; the patient is answering questions about their own upcoming care, not deciding whether to become a patient at all.

Does an AI agent need patient consent before texting about a visit?

Outreach runs inside the consent scope your practice sets, and nowhere else. Hello Patient's agents contact only the patients your team puts on the list, so the boundary is under your control, and a patient who texts STOP or opts out is never contacted again, immediately, with the opt-out recorded. What counts as sufficient consent for your patient population is a question for your own compliance process; practically, the agent cannot reach anyone your team did not put on the list.

How many times does the agent try before it stops?

As many times as your practice configures, and not one more. The agents run the reminder cadence you set across the outreach window, and when that cadence ends without an answer, the outreach closes on its own. There is no indefinite retry and no patient left on a list forever. A STOP or an opt-out ends contact immediately, wherever the cadence stands. The patient who never answered simply arrives at the visit as they would have anyway, and the front desk collects their details at check-in the way it always has.

Can an AI agent collect a patient's insurance details before the visit?

It asks, and it writes down the answer. On a pre-visit call or text, a Hello Patient agent asks about insurance, and whatever the patient says lands as a note on the record for your staff to work from. It can tell the patient whether a plan is accepted. It does not verify coverage, check benefits, or tell the patient what the visit will cost; eligibility stays with your team. It also never asks for a photo of the card, because the agents hold conversations only, with no photos, uploads, or documents involved.

What happens when a patient replies with a question the agent can't answer?

It goes to a person, and the conversation goes with it. When a patient sends something a Hello Patient agent can't handle, a member of your team picks up the thread with everything the patient has said in front of them, so the patient is not asked to repeat anything and the staff member is not guessing at context. The patient asked once and gets an answer once. Everything the agent collected before the handoff still lands on the record the same way.

Can outbound pre-visit intake run across multiple locations?

Yes. Hello Patient's agents run outreach for multi-location groups, and each location's team can hand the agent its own list of booked patients and its own window before the visit. What the agent collects lands in the system the practice runs; the agents work inside ModMed, athenahealth, eClinicalWorks, and many more, and Hello Patient is HIPAA-compliant, SOC 2 Type 2 certified, and signs a Business Associate Agreement with every client. The conversation is the same at every site, which is the point for a group where pre-visit calls currently depend on which front desk had time that week.
